Fill or Refill Your Prescription
Click on the link below to upload your prescription to have your medication filled or refilled using our mobile app. If you’re unable to access the attachment or would like additional assistance for this process, we encourage you to call our pharmacy.
Transfer Your Prescription
If you would like to transfer your prescription from another pharmacy to ours, click on the link below and fill out the Microsoft form. When you have finished answering all of the questions, click “submit” at the bottom of the form. This will send your information directly to our staff to begin the transfer process.

Insurance Information
Sapphire Pharmacy is proud to be in-network with all major medical insurance plans, meaning we will submit claims, pre-authorizations, and any other documentation to ensure that your benefits are utilized to the fullest. For medications that aren’t covered, we also offer affordable prices for cash-pay customers!
